Supported Living Referrals

Myriadlinks Care Solutions welcomes any young person between the age groups of 16 to 21 years olds.

Our referral process usually follows a needs assessment by a person’s local authority. This is followed by an invite to the individual, their family and care manager/social worker, to visit the respective home before continuing with their application. In most cases, we would prefer to offer the individual a choice of the initial meeting so that their requirements are fully considered.

The initial meeting allows us to carry out a comprehensive assessment of the individual’s needs.

The funding authority is sent a written proposal outlining the proposed support package and fee. Once these are agreed, the proposed service user has accepted them, and funding has been agreed, the service user is supported to sign and take up their tenancy following an agreed transition plan to support them to move to their new home.

We will support each new referral to develop:

• a person-centered plan to identify and work towards their aspirations in life
• a structured support plan identifying the details of support services to be provided by Myriadlinks Care Solutions and any other organisation or friends and family
• a health action plan to assist in the promotion of good physical and mental health, ensuring the service user’s well-being and making sure that all appropriate health appointments and checks are planned and monitored.
